ELECTRONIC HEALTH CERTIFICATES - WHAT YOU NEED TO KNOW


Te Office of State Veterinarian, no longer provide small animal CVIs so the only options available are the USDA 7001 form or one of the electronic CVIs.


Regarding the USDA Form 7001, this is a freely accessible form online https://www.aphis.usda.gov/library/forms/pdf/APHIS7001.pdf.


Tis is a generic form without a unique certificate number which is the reason many states do not accept this Paper CVI. So the practitioner must contact the state of destination to determine if this form would be accepted and if it can be emailed. Te contact information for the state of destination state veteri- narian office can be found at https://www.usaha.org/upload/Federal%20and%20State%20Health/ STATE_ANIMAL_HEALTH_OFFICIALS_20.pdf . Te state of destination office will also advise as to what type of signature is acceptable.


For the states where the generic USDA 7001 is not accepted, the only option is to choose from one of the online electronic CVIs (eCVIs). Te three companies approved in Kentucky which provide small animal electronic CVIs are

Note the advantage of these electronic systems, is that the documents are electronically automatically sent to the origin and destination state offices.
