Notably, the 2025 Medicare conversion factor will be reduced by 2.93%, resulting from the expiration of legislative increase valid for 2024 only and a budget neutrality increase of 0.05%. The 2025 conversion factor is $32.36, down from $33.29 in 2024.


CMS’s proposal is expected to result in an estimated aggregate decrease of 2% for interventional radiology, with a 3% cut to non-facility IR settings off set by a 1% increase in facility settings. The reduction in payments is due to how practice expenses contribute to the valuation of services. The cost of equipment and supplies to perform procedures make up the bulk of valuation, with clinical labor and E/M being much smaller factors for specialties like interventional radiology.


In the CY 2025 proposed rule, CMS announced that they will continue to postpone implementation of the updated Medicare economics index (MEI) weights, referencing the American Medical Association (AMA) national study to collect representative data on physician practice expenses, the AMA

Physician Practice Information (PPI) Survey. The PPI Survey launched on July 31st, 2023, and data is anticipated to be shared with CMS in early 2025.


The proposed rule fi nalizes and fully implements the change in the descriptive text of the L041A clinical labor type from angio technician to vascular interventional technologist. Additionally, the pricing of a vascular interventional technologist completes the 4-year phase-in and increase from


CY 2021 value of 0.1 to CY 2025 value of 0.84. The success is due to collaboration between the Society of Interventional Radiology and the American Society of Radiologic Technologists (ASRT) in which radiologic technologist wage and salary survey data was submitted to CMS in 2022. This submission will also fi nalize an increase to the pricing for a mammography technologist from CY 2021 value of 0 to CY 2025 value of 0.79 and for a CT technologist from CY 2021 value of 0 to CY 2025 value of 0.78 in the ruling.


Other proposals of interest include:


• CMS has indicated they will accept the RVS Update Committee (RUC)- recommended values for MRI- monitored transurethral ultrasound ablation (TULSA), percutaneous radiofrequency ablation of thyroid, ultrasound elastography and CT guidance needle placement, and telemedicine E/M services with some revisions.


• CMS propose strategies to improve 90-day global surgical period.


• CMS proposed to extend direct supervision of diagnostic tests through Dec 31, 2025.
