Portal hypertension Session TIPS using fl uoro and ICE guidance

Description: The session begins with an in-depth exploration of TIPS techniques, delving into the intricacies of ICE IVUS, including both standard and IVUS-guided portal access methodologies. Participants will gain

an understanding of fl uoro-guided TIPS access, portography and the anatomical relationships between the hepatic and portal veins. During the hands-on practice component, operators will refi ne their skills in accessing the inferior vena cava and visualizing the hepatic vein in precise alignment with the portal vein. Utilizing ICE guidance, they will adeptly navigate the hepatic vein and advance the TIPS set under fl uoroscopic guidance, culminating in the precise puncture of the portal vein. Once portal vein access is verifi ed, operators will proceed to skillfully advance a guidewire and catheter into the portal vein, followed by meticulous measurement of the tract, dilation, and the placement of a stent-graft.


LEARNING OBJECTIVES:


1. To learn the basics of portal vein access with emphasis on ICE catheter intravascular ultrasonography.


2. To understand the advantages and disadvantages of using ICE vs fl uoroscopy /CO2 for TIPS access.


3. Each operator will have a liver to practice using an ex-vivo perfusion model. The system allows simulation of all the aspects of portal access, tract dilation, measurement and stent graft placement.
