EXPANDED FUNCTIONS


The changes to expanded functions is a proj- ect the Board and the MDA have been work- ing together on for many years. In 2010, the Board and the MDA successfully lobbied the Missouri Legislature to create a law, 332.098 RSMo, requiring that dental assistants and dental hygienists performing expanded functions obtain a permit from the Board authorizing them to do so. The purpose of this change was to create more consistent standards of training for expanded functions and also to implement some type of system to determine how many expanded functions auxiliaries were in Missouri. The law requires that those expanded function permits be renewed every five years.


The next step in this process was to promul- gate a rule determining what the require- ments for obtaining a permit were and what duties could be delegated to a permit holder. The existing expanded function duties were divided into categories.


The first category is Restorative I which would include all the functions delegable to someone who has taken an existing restor- ative expanded functions training course. The duties listed in this category are:  sizing and cementing of prefabricated crowns;


 placing, condensing, and carving amal- gam for Class I, V, and VI restorations;


 placing composite for Class I, V, and VI restorations; and,


 minor palliative care of dental emergen- cies (place sedative filling)


The existing restorative delegable functions were then expanded to include Class II, III and IV restorations. A new Restorative II category was created. The duties listed in this category are:  sizing and cementing of prefabricated crowns;


 placing, condensing, and carving amalgam for Class I, II, III, IV, V, and VI restorations;


 placing composite for Class I, II, III, IV, V, and VI restorations; and,


 minor palliative care of dental emergen- cies (place sedative filling)


The next category was Orthodontics. The du ties listed in this category are:


 preliminary bending of archwire;  removal of orthodontic bands and bonds;


 final cementation of any permanent appliance or prosthesis;


 making impressions for the fabrication of any removable or fixed prosthesis/ appliance; and,

 placement and cementation of orth- odontic brackets and/or bands;


Prosthodontics expanded functions were divided into two categories, Fixed Prosth- odontics and Removable Prosthodontics.


The duties listed in Fixed Prosthodon- tics are:  place retraction cord in preparation for fixed prosthodontic impressions;


or


 extra-oral adjustments of fixed pros- thesis;


 extra-oral adjustments of removable prosthesis during and after insertion;


 final cementation of any permanent ap- pliance or prosthesis; and,


 making impressions for the fabrication of any removable or fixed prosthesis/ appliance.


The duties listed in Removable Prosthodon- tics are:  placement of temporary soft liners in a removable prosthesis;


 extra-oral adjustments of removable prosthesis during and after insertion;


 minor palliative care of dental emergen- cies (place sedative filling); and,


 making impressions for the fabrication of any removable or fixed prosthesis/ appliance.


In reviewing the existing expanded func- tion duties it was noted that one of the duties listed in the old rule was polishing the coronal surfaces of teeth (air polisher). Because Missouri law, 332.093 RSMo, states that the polishing of teeth can be delegated to any dental assistant, it was determined that the Board did not have the authority to restrict any form of polishing to an expanded functions dental assistant. Therefore, the category of Periodontal expanded functions was eliminated.

each expanded function category. The cost of permits is set at $10 per permit to cover the cost of processing and printing the per- mits. As stated in the above mentioned law, expanded function permits will be renewed every five years. The rule requires that the permit holder obtain 16 hours of continuing education every five years to renew their per- mits. Permit holders shall only be required to complete 16 hours of continuing educa- tion regardless of the number of permits the permit holder wishes to renew.


To qualify for and renew a permit, the applicant must provide proof of current certification in Basic Life Support (BLS). It’s important to note that this includes two parts: In Part 1, students complete an online, cognitive portion of the course, either at the workplace or anywhere with Internet access. When students finish the online portion, a certificate of completion will be available for them to print. The certificate is required to complete Part 2 at an authorized American Heart Association (AHA) Training Center. For Part 2, students complete the skills vali- dation test with an American Heart Associa- tion Instructor. Upon successful completion of both parts, students will be issued an AHA BLS-Healthcare Provider Course Completion Card.
