It highlights the value of pharmacy: Simple, but


impactful. One of pharmacy’s biggest value


to healthcare is the ability to provide quality follow-up and monitoring. Check out the article in this issue Better Pharmacies Save Lives (pg. 19). All pharmacies are NOT the same. And this program is showing a pharmacy equipped with a community health worker (or two or three or…) are a force to be reckoned with.


It takes a village. T is phrase applies to achieving


whole patient and community care as much as it does to raising children. Healthier patients and healthier communities include caring for more than the pharmacy or the primary care physician or the hospital silos. It includes social support, which can be expanded through CHW’s in pharmacies. If 90% of Americans lived within fi ve miles


of a community pharmacy, imagine if those pharmacies were also equipped to serve those communities in addressing health disparities and connecting the needs of patients with resources. MPA, CPESN MO, and CPESN Health Equity are working to show the impact of partnering preventive and primary care expansion with social resource expansion. So, what’s next? Getting the word


out! Missouri is leading the way for advanced pharmacy teams – and other states are paying attention. T e CPESN Health Equity team is supporting other local networks, based on the leadership and vision for CPESN MO and MPA, to provide similar programs in other states. And in Missouri, the next phases of the program launch this month (July). T is is an enhancement of the current program – it’s time to show the impact of the pharmacy team on the community, whether they use the pharmacy or not for medications.
