There are many organiza- tions out there addressing these social determinants, but we’re care coordinating into a system that is not integrated, and into a system  all the needs of the people that are referred to them. But there’s this tremendous opportunity for better integration. And that comes with better sharing of data and the technology behind that and the lining incentives between PEARS and providers. Those are the keys to tru- ly expanding and scaling this concept of addressing the social deter- minants and truly impacting health outcomes and lives.


Joe Gaudio, your leadership style is Servant Leader. Would you explain to the readers what that means? And how you im- plement that style with your team of Doctors, Nurses, Social Workers, Therapists, Community Health Workers, and other healthcare liaisons? From a Managment and a personal perspective I have been heavily  is a leader in the organizations, leader in your community, focuses primarily serving others, whether it’s individuals in the community and those individuals you work with on a day to day basis.
