Early Registration Deadline: January 9, 2015 After this date, all registration categories increase by $25 per attendee.
Cancellations: All cancellation requests must be made in writing. Any cancellations prior to January 9, 2015 are subject to a $50 processing fee per registrant. Refunds will not be granted, for any reason, after 11:59pm on January 9. Registrations may be transferred at any time without penalty.

HOTEL RESERVATIONS
For hotel reservations contact the Holiday Inn Select Executive Center at 800-HOLIDAY or at 573-445-8531. Reservations must be made by December 15, 2014. Room rate is $96.95 plus tax, per night. When reserving room state you are with the Missouri Dental Association or MDA. The Holiday Inn Select is located at 2200 I-70 Drive SW, Columbia, Missouri. Check in time is 4pm. Check out time is 11am.
